Town Meeting Forms

Forms are included here for your convenience. You may use any form that includes all the necessary elements for your purpose.

• Citizen Petition for a Warrant Article (JPG): Use this form if you wish to require the Select Board to place your article on the Town Meeting Warrant. Signatures of at least ten Sherborn registered voters are required. Petitioners are advised to include at least 25% more signatures than needed and the petition must be turned into the Town Clerk's Office by the deadline set by the Select Board for when the warrant closes. The first voter signature on the petition will be listed as the representative for the warrant article in the advisory report distributed for Town Meeting. The Town Clerk's Office certifies all valid signatures. Electronic copies of the petition cannot be accepted nor are electronic or docusign signatures. All signatures must be regular ink signatures.
• Citizen Petition for a Special Town Meeting (PDF): Use this form if you wish to require the Select Board to call a Special Town Meeting (must be held within 45 days of receipt). Requires signatures of at least two hundred Sherborn registered voters. Petitioners are advised to include at least 25% more signatures than needed. Must be turned into the Select Board Office to start the clock. Town Clerk's Office certifies all valid signatures.
• Request for Town Meeting Presentation Time (PDF): Use this form if you are a proponent or opponent of a warrant article to request the Moderator to allow you a five minute presentation at Town Meeting. If you are not granted presentation time, you will be limited to the standard two minute comment time. Must be done in advance of Town Meeting.
• Motion/Amendment Form (PDF): Use this three-part form when you plan to offer a motion or an amendment to a motion. Give one copy to the Town Clerk and one copy to the Moderator before offering your motion or amendment. Alternately you may print/make three copies of your own written motion or amendment. These forms are also available at Town Meeting near the check-in tables and at the foot of the stage.
